Gov.UK and the Department of Health and Social Care have produced a leaflet of guidance on how to care for someone when they leave hospital.
The leaflet is available in English, Arabic, Bengali, Chinese (Simplified), Chinese (Traditional), Gujarati, French, Polish, Portuguese, Punjabi, Spanish and Urdu.
