The Department of Health and Social Care have produced a leaflet for patients prior to their discharge from hospital to go to further non-acute bedded care, such as a care home.
This leaflet is available in Arabic, Bengali, Chinese - Simplified, Chinese - Traditional, French, Gujarati, Polish, Portuguese, Punjabi, Spanish and Urdu.
The leaflet is also available in Easy Read, in English.
An editable PDF version of the leaflet allows specific contact details to be added in case patients need to speak to someone after leaving hospital.
